Use this form to request a copy of a Wisconsin Rapids Police Department Incident Report.  When submitting this request, please keep the following criteria in mind: 
 
If you are requesting an accident report please us the Accident Report Request form.
  • The Police Department charges $0.50 per page for police reports.  Other charges or prepayment may be required depending on your request.
  • Juvenile information is required to be kept confidential under Wisconsin State Statutes 48.3936(1) and 983.396; and will not be released.  If you need a report pertaining to a juvenile, please follow this link for separate instructions.
  • Incident Reports requested by victims pertaining to identity theft, financial crimes, and victim-sensitive crimes may require proper identification for full release.  The department prefers that requesters pick up the reports and provide proof of identity at the time of pickup.
  • Wisconsin Statutes 146.81-84 and 51.30(4) states that a persons health care and treatment records are private and confidential. 

Because this request is being made electronically, please provide as much contact information as possible so that we can appropriately process your request.  While none of the information is required, a contact method and information must be provided for notification of the availability of the report.
